Multiprofessional action in prevention of skin injuries

Objective: The objective was to prevent, identify and stratify possible skin lesions by tracing the risk profile of the patients and their needs, as well as the factors that influenced the onset of lesions, and which may be linked to the patient's clinical conditions or the nursing care directly. Result: The good practices implemented favored the management of better results, with evidence of effectiveness and adherence to prevention practices, cost reduction, in addition to the recognition and motivation of our professionals. In the second half of 2016, we had 58 reported skin injury events (average of 10 events / month); In 2017, we had 66 reported skin injury events (average of 06 events / month); and in 2018, we had 20 reported skin lesion events (average of 02 events / month), a result compared from 2016 to 2018, we had more than 80% drop in the number of events. Since 2017, this institution has gone through the certification process, and in April 2019 was recertified for developing best practices related to injury prevention receiving the Diamond category. Conclusion: Our experience made it possible to reduce 80% of skin injury events, which highlights the importance of the multiprofessional team in the evaluation, planning, prevention of skin lesions. We also noticed that the involvement of the patient and family in educational measures and in the elaboration of the care plan was of great relevance for the continuation of good practices after patient discharge.
